You may download a PDF copy of the appropriate registration form or scholarship form and mail them with the deposit. Your place will not be reserved until the deposit is received and the application approved. When your application is approved, you will receive a letter with the approval and additional information forms such as the medical form, name tag form, etc. Note: some folks have problems printing these forms (and other PDF files) from their browsers. If you are experiencing this problem, it is normally because you do not have a recent version of Adobe Reader or some other program on your computer. To print out these forms when you are experiencing this problem, please save the file to your computer and then open it up directly (not in your browser) with Adobe Reader or some other PDF reading program. |

Note 1: These forms can be filled out and printed on your computer using
Adobe Reader or another PDF reader. If you want to save an electronic
copy of the form, you must use a PDF reader other than Adobe since it
will not save the filled out form.
Note 2: The Health History Form 1 to be filled out by parent/guardians and
the Health Care Recommendation Form 2 to be filled out by the physician. |
